What applies in healthcare
The Labour Act applies to private Spitex (home care) organisations, practices and care homes. For hospitals, clinics and care homes, Ordinance 2 to the Labour Act (ArGV 2) contains special provisions, for example on night and Sunday work.

Irregular shifts, night and weekend work are normal in healthcare. That is exactly why working time must be recorded without gaps. Public-sector institutions are often subject to cantonal staff regulations.
On-call duty (Pikett)
On-call duty on the premises counts in full as working time. For on-call duty from home, the time spent on the call-out including travel counts. Clocking every call-out gives you properly documented billing.
Rest after a shift
As a rule, at least 11 hours of rest are required between two shifts. The history shows when one shift ended and the next one began.
